Man, where to start? In no particular order:

• Unorganized navigation - items within a section that aren't relevant
• Unorganized content - improper topic/folder structure
• Improper URL structure - making it difficult to track conversion activity
• Non-functional hero image sliders - lacking call to actions, don't link to a lower level page and/or having more than 7 or 8 slider images
• Stale, outdated, unpersonalized page copy - or just blocks of unnecessary text that nobody is ever reading
• Unnecessary clutter - unimportant page elements
• Improper page hierarchy - (ex: making everything the same size font, or putting non-important elements above key business drivers)
• Video's that play upon page load - and that are difficult to close
• Cluttered VDP's that still have a dozen call to actions and/or don't remain a constant grid - switching from full width, to two column layouts - or even three columns
• Any type of tabbed or accordian content

I'll add more - just scratching the surface.
